Position Overview

As a home health PT, you will help patients regain mobility, strength, range of motion, and safety so they can stay independent at home. You will report to our Director of Nursing, and Administrator.

Key Responsibilities

Provide physical therapy services ordered by a physician, physician assistant, or advanced registered nurse practitioner that can be safely delivered in the home

Perform evaluations and regular or OASIS assessments accurately, following the OASIS Implementation Manual

Assist the ordering practitioner by applying diagnostic and prognostic tests of muscle, nerve, joint, and functional ability

Evaluate equipment needs and teach patients and families how to use and care for therapy devices

Document your visits and findings in the clinical record, and report patient responses, changes in condition, and any deviation from the plan of care to the ordering practitioner

Teach family members and other care team members, including Home Health Aides, the parts of the therapy program they will support

Make supervisory visits to assigned Home Health Aides and Physical Therapist Assistants (PTAs)

Take part in in-services and discharge planning

Follow HIPAA, agency policies, and Florida and federal regulations

Qualifications & Requirement

Graduate of an approved Physical Therapy school

Current, active Physical Therapist license in the State of Florida

At least 1 year of experience as a Physical Therapist

Ability to meet the health and physical requirements of a home health agency

Clear spoken and written English, including on the phone and with patients

Reliable personal transportation and a valid driver's license, with a clean driving record

Current auto insurance that meets Florida requirement

Able to transfer, lift, or assist patients averaging 160 pounds, with or without mechanical aids

Able to spend about 80% of the workday standing or moving about

Able to walk, climb stairs, stoop, twist, bend, and squat

Preferred

Home health or OASIS experience

Bilingual in English and Spanish

Pay: $48.00 - $53.00 per hour
