Visiting Rehab and Nursing Services (VRNS) is seeking a skilled Physical Therapist (PT) to join our growing home health team in Wakefield, Massachusetts. Your Role in Home Health
As a Physical Therapist (PT), you’ll provide individualized therapy in patients’ homes, helping them improve strength, balance, mobility, and functional independence. You’ll develop treatment plans based on each patient’s needs and goals while building meaningful connections through one-on-one care.
Key Responsibilities
- Complete physical therapy evaluations and develop individualized treatment plans
- Provide therapeutic exercises and interventions to improve strength, balance, mobility, and function
- Address gait training, transfers, and safe mobility
- Assess home safety and recommend adaptive equipment when appropriate
- Educate patients and caregivers on treatment plans and home exercise programs
- Monitor patient progress and adjust interventions as needed
- Collaborate with physicians and other healthcare professionals
- Maintain accurate and timely clinical documentation

Requirements:
Need to Have:
- Current Massachusetts Physical Therapist (PT) license
- Minimum of one (1) year of clinical experience
- Afternoon availability
Nice to Have:
- Previous home care experience
- Monday–Friday availability
- Weekend availability
